The Reports section provides tabular data views of information collected from participants. Each report can be filtered, sorted, grouped, and exported to Excel or CSV. Reports are found in the session sidebar under Reports.

The Health History Report shows medical and vaccination data collected from participants' health forms.

The Emergency Contacts Report shows each participant's primary emergency contact information.

The Forms Report shows form field data for a selected form type. Unlike the other reports, its columns are dynamic — they depend on which form type you select.
A Form Type dropdown appears in the top right of the report header. Select a form type to load its data. The grid populates with one column per field from that form, plus participant name columns.
If no form type is selected, the grid shows "Select a form type to view data."
Columns are generated from the form fields of the selected form type. Each field in the form becomes a column. Column names match the field labels as configured in Form Types.
All three reports share the same grid capabilities:
Type in the search box (top right of the header) to filter visible rows. The search highlights matching text.
Drag any column header to the group panel (at the top of the grid) to group rows by that column. Right-click column headers for grouping options.
Click Column Chooser in the action menu to show or hide columns. Many useful columns (date of birth, insurance, phone numbers) are hidden by default but available here.
Click the Export button in the grid toolbar to download the current view as an Excel file. You can export all rows or only selected rows.
Each report's action menu also includes an Export to CSV option (CSV file icon). Unlike the Excel export, the CSV export mirrors exactly how you've set up the grid:
The file downloads as {Session Name} - {Report Name} {date}.csv (for example, Summer Camp 2026 - Health History 2026-06-23.csv). Exporting to CSV requires the same role as viewing the report.
Participants must have submitted their health forms for data to appear. If the session has participants but the report is empty, check whether any forms have been submitted. You may also need the Participant Form Viewer role or higher.
Select a form type from the dropdown in the top right. If no form types appear, no forms have been assigned to this session, or no participants have submitted forms.
Q: How do I see emergency contact information for all participants? A: Go to Reports → Emergency Contacts in the session sidebar. The report shows each participant's primary emergency contact. You can filter, sort, and export the data to Excel.
Q: How do I see vaccination records for all participants? A: Go to Reports → Member Health History. The report shows vaccination history and medical equipment (inhalers, epinephrine injectors). Use the Column Chooser to show additional fields like date of birth and insurance information.
Q: How do I export a report to Excel? A: On any report page, click the Export button in the grid toolbar. You can export all rows or just the selected rows.
Q: How do I export a report to CSV? A: Open the report's action menu and choose "Export to CSV." The CSV includes all rows that match your current filters and sort order — across every page, not just the page on screen — using the columns currently shown on the grid. Use the Column Chooser first if you want to add or remove columns before exporting.
Q: The Forms Report is empty — what do I do? A: Select a form type from the "Form Type" dropdown in the top right of the report. The columns and data change based on which form type you choose.
Q: Can I filter the report to show only certain participants? A: Yes — use the filter row below the column headers to filter by any column value, or use the header filter (funnel icon in column headers) for dropdown-based filtering. You can also use the search box in the top right.
