Accepting Invitations & Managing Multiple Family Members

What is an Invitation?

Each participant—whether a child or adult—receives a unique invitation email. This invitation gives access to upload health forms and manage event details.

The email comes from no-reply@app.healthforms.io and contains a "Get Started" button for easy invitation acceptance.

Can’t find the invitation? Check your spam/junk folder or search for no-reply@app.healthforms.io.

One Account Per Family

Only one account is needed per family. A parent or guardian can manage multiple family members (e.g., children or another attending adult) from a single login.

If You Have Multiple Invitations:

  1. Start with any one of the invitations.

  2. Click Get Started to create your HealthForms.io account.

  3. The Family Portal will walk you through adding the participant from that invitation.

Once you're logged in and the first participant is added, you're ready to accept the other invitations.


Accepting Additional Invitations

There are 3 ways to add more family members to your account.

Invitation Email

  1. Open the next invitation email and click the Get Started button.
  2. You’ll be prompted to link (or add a new Family Member) the event participant to your Family Portal.
  3. Repeat for each additional family member.

Use Invitation Code

  1. Log in to the Family Portal (https://app.healthforms.io/portal)
  2. Click the barcode button (top right of the navigation bar).
  3. Enter the invitation code
  4. You’ll be prompted to link (or add a new Family Member) the event participant to your Family Portal.

Pending Invitations

If additional invitations have been sent to your user's email address, a list of pending invitations is available in the Family Portal Side Panel.
  1. Log in to the Family Portal (https://app.healthforms.io/portal)
  2. Click on the Side Panel button (top right of the navigation bar).
  3. Click on the pending invitation you would like to link to your Family Portal profile.
  4. You’ll be prompted to link (or add a new Family Member) the event participant to your Family Portal.

Each person will appear in the Family panel of your Family Portal, where you can manage their forms and profiles individually.

What to Expect in the Family Portal

Once all invitations are accepted and linked:

  • You’ll see each family member listed separately in the portal.

  • Select a person’s name to view or upload their health forms.

  • Each profile will show relevant events, deadlines, and submission status.


Troubleshooting Tips

  • Don’t see an event listed? Try re-clicking the invitation link to ensure the person is linked, or go to https://app.healthforms.io/account/invitations to manually enter the invitation code in the email.

  • Family Member Not Showing Any Events? This usually means their event invitation hasn’t been accepted yet. Please check your email (including spam/junk folders) for the original invitation and click the Get Started button. If you can’t find it, contact your event organizer to have the invitation resent.
