Family Member Profile

The Family Member Profile page shows all information for a single family member: their upcoming and past sessions, contact details, emergency contact, insurance information, and a history of submitted forms.

Navigate here by clicking a family member's card on the Dashboard, or clicking their name elsewhere in the portal.


The header shows:

  • Full name of the family member
  • Email address (if on file)
  • Phone number (US format, if on file)
  • A Back to Dashboard button (top right) to return to the Dashboard

Sessions Section

The Sessions section lists every event the family member is enrolled in. Each session appears as a clickable card.

Session Card Contents

Element Description
Session name Name of the event
Date range Start and end dates of the session
Due date Form submission deadline (shown in red if within 10 days)
"Action Required" label Shown in red if the member has rejected forms; shown normally if required forms are still pending
Chevron icon Indicates the card is clickable

Card colors: | Color | Meaning | |-------|---------| | Red/danger border | Member has rejected forms in this session | | Green/success border | All required forms have been submitted | | Blue/primary border | Forms pending (no rejection) |

Click any session card to go to the Session Forms page for that event.

Show More

The page initially shows 5 sessions. Click Show More to load 5 additional sessions at a time.

If no sessions are found, the section shows: "No sessions found"


Personal Information Section

Below the Sessions section is the Personal Information section with three information cards.

Contact Card

Shows the family member's contact information:

  • Email address (or "Not Set" if blank)
  • Phone number (or "Not Set" if blank)

Click Edit (pencil icon) to open the Edit Contact popup to update the email address and phone number.

Emergency Contact Card

Shows emergency contact details, split into Primary and Alternate sections:

Primary contact:

  • Name and relationship (e.g., "Jane Smith — Mother")
  • Mobile phone
  • Other phone (if provided)
  • Address (if provided)

Alternate contact (shown only if an alternate name or phone is on file):

  • Alternate name
  • Alternate phone

Click Edit (pencil icon) to open the Edit Emergency Contact popup where you can update all emergency contact fields.

Insurance Information Card

Shows the family member's insurance:

  • Insurance Company ("None" if no insurance is on file)
  • Policy Number ("None" if no insurance)
  • Last Updated date (or "Never" if never entered)

Click Edit (pencil icon) to open the insurance card editor where you can upload or update insurance card information.


Submitted Forms Section

If the family member has previously submitted any forms, a Submitted Forms section appears at the bottom of the page. Each submitted form appears as a row showing:

Column Description
Form type name The type/name of the form
Upload date "Uploaded [date]"
Signed date "Signed [date]" (if applicable)
Download button Click to download a copy of the submitted form

The section initially shows 5 forms. Click Show More to load 5 more at a time.

This section does not appear at all if the member has no previously submitted forms.


Common Issues

The session I'm looking for doesn't appear

Click Show More to load additional sessions beyond the first 5. Sessions include past sessions — look for older entries if the event has already ended.

A submitted form is not in the Submitted Forms section

Only forms uploaded through the portal appear here. If the form was submitted another way (e.g., mailed or faxed), it may not show. Contact the organization or visit https://support.healthforms.io for assistance.

My emergency contact information is wrong

Click the Edit button on the Emergency Contact card to update all emergency contact fields. Changes are saved immediately.

Q: How do I edit a family member's contact information? A: Go to the family member's profile page, scroll to the Personal Information section, and click the Edit button on the Contact card. Update the email or phone number and save.

Q: How do I update emergency contact information? A: Go to the family member's profile, find the Emergency Contact card in the Personal Information section, and click Edit. You can update the name, relationship, phone numbers, address, and alternate contact.

Q: How do I update insurance information? A: Go to the family member's profile, find the Insurance Information card, and click Edit. You can upload a new insurance card or update the insurance company and policy number.

Q: How do I download a previously submitted form? A: Go to the family member's profile and scroll to the Submitted Forms section. Find the form and click the Download button to save a copy.

Q: How do I see what sessions a family member is enrolled in? A: Go to the family member's profile. All sessions are listed in the Sessions section. Click any session card to view the forms for that session.
