The Session Forms page shows all the health forms a family member must complete for a specific session (event). From here you can see each form's status, submit forms, download blank copies, and request fax cover sheets.

If the session has a description or a support email address configured, a "Show Event Details" toggle link appears below the header. Click it to expand a card showing:
Click "Hide Event Details" to collapse it again.
When the session has at least one form assigned, a progress banner shows:
If any forms can be acted on (i.e., are available to upload or fill), a blue information callout appears:
"Click the action button on each form below to upload or fill it out. Forms marked Required must be completed before the session. Need help? Contact [support email]."
If any forms in this session are grouped into a combined-submission packet, those groups appear first as packet cards. A packet card shows:
Click Get Started to open the packet wizard, which walks you through all forms in the packet in a single flow.
Forms not in a combined packet are listed individually below any packet cards. Forms are sorted in this order:
Each form card shows:

After clicking Replace Submitted Form, a dialog appears: "Are you sure you want to resubmit and reset the status of this form? This may require the form to be reviewed again." Click OK to continue, Cancel to abort.
If you have previously submitted the same type of form, a "Select existing form" dropdown appears alongside the action button with an "or" separator. You can:
For forms that are uploaded (not online-fill), a three-dot menu (⋮) appears in the top-right corner of the card. It contains:

Some forms require a doctor's signature. If your doctor prefers fax, you can download the form with a fax cover sheet, which lets the doctor fax the signed form directly to HealthForms.io.
Click Download With Fax Cover Sheet in the three-dot menu (⋮). A modal appears:
"Download this form with a fax cover sheet to allow your healthcare provider to fax the signed form to HealthForms.io. Please see the instructions on the fax cover sheet or go to https://support.healthforms.io/portal/en/kb/articles/faxing-health-forms."
Click Download to download the PDF. Click Close to cancel.
If the session has no forms assigned yet, the page shows:
"Forms Are Being Defined"
This means the organization hasn't finished configuring the session's forms. Check back later or contact the organization.
The form may be in a "waiting" state — it's required but not yet available for submission (for example, it might depend on another form being submitted first). The status badge on the card will reflect this.
After uploading, the form typically moves to a "Pending Review" or similar status while the organization reviews it. This is normal. You'll receive an email if the form is approved or rejected.
Click the action button on the form card (it now reads Replace Submitted Form). Confirm the dialog, then go through the submission wizard again with the corrected version.
Q: How do I submit a health form for a session? A: Go to the family member's profile, click the session, then find the form card and click "Fill Out Form" or "Upload Form." Follow the step-by-step wizard to complete and submit the form.
Q: How do I know which forms are required? A: Each form card shows a "Required" badge (red) or "Optional" badge (grey). Required forms must be submitted before the session. The progress banner at the top also shows how many required forms are still outstanding.
Q: A form I submitted was rejected — what do I do? A: Click the action button on the rejected form card. A dialog will ask you to confirm resubmission. After confirming, go through the Form Submission Wizard again to submit a corrected version.
Q: How do I download a blank copy of a form? A: Click the three-dot menu (⋮) in the top-right corner of the form card, then choose "Download Blank Form." This downloads the blank form template as a PDF.
Q: How do I submit a form via fax? A: Click the three-dot menu (⋮) on the form card and choose "Download With Fax Cover Sheet." Print the downloaded PDF and bring it to your doctor. The fax cover sheet includes instructions for the doctor to fax the signed form back to HealthForms.io.
Q: What is a form packet? A: A form packet is a group of forms that must be submitted together in one combined flow. If your session uses packets, a packet card appears at the top of the page. Click "Get Started" to complete all the forms in the packet at once.
