How to Manage Your Account

Update your personal information, email, password, and multi-factor authentication (MFA) settings.

Overview

What it is: Account management lets you view and update your personal information and security settings.

Who uses it: All HealthForms.io users.

Why it matters: Keeping your information current ensures you can log in, reset your password, and use MFA correctly.

Accessing Your Account

How to Get There

  1. Log in to HealthForms.io at https://app.healthforms.io
  2. After logging in you are taken directly to the Manage your account page
  3. You can return to it any time by navigating back to https://app.healthforms.io/account

What You'll See

The Manage your account page is divided into four cards:

Card What's in it
HealthForms.io Apps Links to launch Portal or Manager (if you have access)
User Information Your name and email address
External Login Providers Link or unlink your Google account
Security Change your password
Multi-Factor Add/remove Authenticator App, Text Message, Recovery Codes, and Default Method

User Information

View Your Name and Email

The User Information card shows your current:

  • Name: First and last name
  • Email: Your current confirmed email address
  • If a change is pending, you'll also see your unconfirmed new email with options to cancel or resend the confirmation email

Update Your Name or Email

  1. In the User Information card, click Update
  2. You are taken to the Update Personal Information form, which has:
    • First Name
    • Last Name
    • Email (labeled "Email" — entering a new value starts the change process)
  3. Update the fields you need
  4. Click Update

If you changed your email:

  • You are shown a confirmation message asking you to check your email
  • A confirmation link is sent to your new email address
  • Click the link in that email to confirm the change
  • Until confirmed, your old email remains active and the new one shows as Unconfirmed on the account page
  • You can Cancel or Resend Email from the account page while the change is pending

Important: After confirming, you must use your new email to log in going forward.


External Login Providers

The External Login Providers card shows your Google account connection.

If no Google account is linked:

  1. Click Continue with Google (the Google sign-in button)
  2. Sign in to your Google account
  3. Google is now linked — you can log into HealthForms.io using Google instead of your password

If a Google account is already linked:

  1. Click Unlink Google Account
  2. Google is removed from your account
  3. You must use your email and password to log in going forward

Note: Make sure you know your HealthForms.io password before unlinking Google, so you don't lose access.


Security — Change Password

The Security card has a single action: Change My Password.

Steps to Change Your Password

  1. Click Change My Password
  2. You are taken to the Change Password form with three fields:
    • Current Password — your existing password
    • Password — your new password
    • Confirm Password — re-enter your new password
  3. Fill in all three fields
  4. Click Change Password

Password requirements (minimum 8 characters):

  • At least one uppercase letter (A-Z)
  • At least one lowercase letter (a-z)
  • At least one number (0-9)
  • At least one special character (!@#$%^&*)

Click Cancel to go back to the account page without saving.

If you don't know your current password, use Forgot Password from the login page instead.


Multi-Factor Authentication (MFA)

The Multi-Factor card is where you manage all MFA settings. You can have one or both methods enabled simultaneously.

Authenticator App

If not set up — you will see: Add Authenticator App
If already set up — you will see: Remove Authenticator App (disabled if it's your only MFA method)

Add Authenticator App

  1. Click Add Authenticator App
  2. You are taken to the Setup Two Factor Authenticator page with three steps:
    • Step 1: Download an authenticator app
    • Step 2: Scan the QR code shown on screen, or manually enter the key displayed below it
    • Step 3: Enter the 6-digit code your app generates and click Verify
  3. Your Recovery Codes are displayed — save these immediately
  4. Click Finish

Remove Authenticator App

  1. Click Remove Authenticator App
  2. Confirm by clicking Remove on the confirmation page
  3. Click Cancel to go back without removing

You cannot remove a method if it is your only MFA method. Add the other method first.


Text Message (SMS)

If not set up — you will see: Add Mobile Phone
If already set up — you will see: Remove Mobile Phone (disabled if it's your only MFA method)

Add Mobile Phone

  1. Click Add Mobile Phone
  2. You are taken to the Update Multi-Factor Phone Number form:
    • Country Code (read-only, pre-filled)
    • Phone Number — enter your mobile number
  3. Click Verify
  4. A verification code is texted to your phone
  5. You are taken to the Verify Phone Number page:
    • Enter the code texted to you
    • Click Verify, or click Resend Verification Code if you didn't receive it
  6. Your Recovery Codes are displayed — save these immediately
  7. Click Finish

By adding a mobile phone you agree to receive account security text messages from HealthForms.io, including 2FA codes.

Remove Mobile Phone

  1. Click Remove Mobile Phone
  2. Confirm by clicking Remove
  3. Click Cancel to go back without removing

Recovery Codes

Recovery codes are shown whenever you add a new MFA method. They allow you to log in if you lose access to your authenticator app or phone.

If you already have MFA enabled, you can regenerate recovery codes at any time:

  1. Click Regenerate Recovery Codes
  2. New codes are displayed — save them immediately
  3. Click Finish

Important: Regenerating codes invalidates all previous codes. Save the new ones somewhere safe — a password manager is recommended.

Recovery codes appear as a list of short codes. Each code can only be used once. Once all codes are used up, regenerate a new set.


Default MFA Method

This option only appears when you have both Authenticator App and Text Message enabled.

It lets you choose which method is used by default when you log in:

  • Authenticator App
  • Text Message
  1. Select your preferred method from the dropdown
  2. Click Update

You can always switch to the other method on the login screen using the alternate option link.


HealthForms.io Apps

If your account has access to multiple HealthForms.io applications, they are listed at the top of the account page with a Launch button and brief description for each:

  • Portalhttps://app.healthforms.io/portal
  • Managerhttps://app.healthforms.io/manage

Click Launch to open that application in a new tab.


Common Issues

Issue: Email Change Confirmation Not Arriving

Solutions:

  1. Check your spam/junk folder for an email from HealthForms.io
  2. Go back to the account page and click Resend Email next to the unconfirmed email
  3. If you used the wrong new email, click Cancel and start the change again with the correct address

Issue: Can't Remove MFA Method

Symptoms: The Remove button is grayed out / disabled

Cause: You cannot remove your only MFA method. You must have at least one method active at all times once MFA is set up.

Solution: Add the other MFA method first, then remove the one you don't want.

Issue: Phone Verification Code Doesn't Arrive

Solutions:

  1. Wait up to 2 minutes — SMS can be slow
  2. Click Resend Verification Code on the verification page
  3. Check you entered the correct phone number
  4. Try using an Authenticator App instead

Issue: Can't Change Password — Don't Know Current Password

You cannot change your password from the account page without knowing your current password.

Solution: Log out, go to the login page, and use Forgot your Password? to reset it via email.


Q&A

Q: How do I update my name? A: Log in and go to your account page. In the User Information card, click Update, edit your First Name or Last Name, then click Update.

Q: How do I change my email address? A: Log in and go to your account page. In the User Information card, click Update, enter your new email, and click Update. A confirmation link will be sent to the new address — click it to complete the change.

Q: How do I change my password? A: Log in and go to your account page. In the Security card, click Change My Password, enter your current password and new password, then click Change Password.

Q: How do I add two-factor authentication? A: Log in and go to your account page. In the Multi-Factor card, click Add Authenticator App or Add Mobile Phone and follow the setup steps. See MFA Setup Guide for details.

Q: How do I get new recovery codes? A: Log in and go to your account page. In the Multi-Factor card, click Regenerate Recovery Codes. Save the new codes immediately — old codes are invalidated.

Q: Can I link my Google account to HealthForms.io? A: Yes. Log in and go to your account page. In the External Login Providers card, click Continue with Google to link your Google account.
