How to Log In

Log in to access your HealthForms.io account and use Portal or Manager applications.

Overview

What it is: Logging in verifies your identity so you can access HealthForms.io apps safely.

Who uses it: Anyone with a HealthForms.io account.

Why it matters: Login keeps your data secure by ensuring only you can access your account.

Quick Start

  1. Go to the HealthForms.io login page at https://app.healthforms.io
  2. Enter your email and password
  3. Click Sign In
  4. If you have MFA enabled, enter your 2FA code
  5. You're logged in!

Expected result: You see the main dashboard/home page of the app.

Time: 2-3 minutes (or 1 minute if you've done it before)

Step-by-Step Guide

Step 1: Go to Login Page

Navigate to the login page in your browser:

  • Portal: https://app.healthforms.io/portal
  • Manager: https://app.healthforms.io/manage

Note: Bookmarking the login page makes it easier next time.

Step 2: Enter Your Email

In the Email field, type the email address associated with your HealthForms.io account.

Example: john.smith@example.com

Tip: Make sure you enter it exactly as it was registered. Email addresses are case-insensitive, but spaces matter.

Step 3: Enter Your Password

In the Password field, type your password.

What to know:

  • Passwords are case-sensitive (uppercase vs. lowercase matters)
  • Passwords must be at least 8 characters
  • The password field hides what you're typing for security

Tip: If you're not sure of your password, use Forgot Password to reset it.

Step 4: Complete Security Verification (Cloudflare Turnstile)

After entering your password, you may see a Cloudflare Turnstile verification box.

What it is: This is a security check to protect your account from unauthorized access. It's similar to a CAPTCHA but faster and less intrusive.

What to do:

  • If you see a checkbox: Click the checkbox to verify you're human
  • If you see a puzzle/challenge: Complete the simple challenge (usually identifying images)
  • If you see nothing: The verification may happen automatically in the background

This process usually takes 1-3 seconds.

Step 5: Click "Login"

Click the Login button to log in. The button is disabled until the Cloudflare Turnstile verification completes in the background.

Wait: It may take 2-5 seconds to process.

Step 6: (If MFA Enabled) Enter 2FA Code

If you've set up Two-Factor Authentication (2FA), you are taken to the MFA verification page. The heading and instructions shown depend on which method is your default.

Enter your 6-digit code and click Verify. You can also check Remember this device to skip MFA verification on this device in the future.

Having problems? The page shows a "Having problems?" section with links to:

  • Use a text message — switch to SMS if your default is authenticator app
  • Use your authenticator app — switch to authenticator if your default is SMS
  • Use a recovery code — use a one-time backup code if you can't access your usual method

Click Cancel to return to the login page.

Step 7: Confirm Email (First Time Only)

On your first login, you may be asked to confirm your email address. HealthForms.io sends a 6-digit code to your email.

  1. Check your inbox for an email from no-reply@app.healthforms.io
  2. Enter the 6-digit code on the confirmation page (the form submits automatically when all 6 digits are entered)
  3. If the code expires (countdown shown on page), a Resend Code link appears after 60 seconds
  4. If you entered the wrong email, click the [Change] link to correct it

Check your spam folder if the code doesn't arrive within a few minutes.

Step 8: Accept Terms (If Applicable)

If HealthForms.io has updated its Terms of Use or Privacy Policy, you may be asked to review and accept the changes.

  1. Read the summary of what changed
  2. Click View Terms of Use or View Privacy Policy to read the full documents (optional)
  3. Click Accept Terms to agree and continue
    • If acceptance is required: the alternative is Logout
    • If acceptance is optional: the alternative is Decide Later

Logging In with Google

If you have a Google account, you can log in with Google instead of a password:

  1. On the login page, click Sign in with Google
  2. A Google login popup appears
  3. Enter your Google email and password
  4. Google confirms your identity
  5. You're logged into HealthForms.io!

Advantages of Google Login:

  • ✅ One fewer password to remember
  • ✅ Google's security protects your account
  • ✅ Faster login process

Note: You can only use Google login if it was set up when your account was created.

"Remember Me" Option

The login page has a Remember Me checkbox:

  • Checked: Your computer will remember you. Next time, you may not need to enter your password
  • Unchecked: You'll need to log in again next time

Security Tip: Only use "Remember Me" on personal computers, not shared or public computers.

Common Issues

Issue: "Invalid Email or Password"

This error appears when:

  • Email address doesn't exist
  • Password is incorrect
  • Email or password was entered with a typo

Solutions:

  1. Check for typos in your email
  2. Check CAPS LOCK is not on
  3. If you're sure about the password, use Forgot Password to reset it
  4. Try a different browser

Issue: "Account Locked"

Your account locks after 5 failed login attempts for security. It unlocks automatically after 30 minutes.

Solutions:

  1. Wait 30 minutes and try again
  2. Use Forgot Password to reset and unlock your account
  3. Open a support ticket at https://support.healthforms.io if you need immediate access

Issue: "Please Confirm Your Email"

This appears if you haven't verified your email address yet.

Solution:

  1. Check your email for a confirmation message from HealthForms.io
  2. Click the link in that message
  3. Try logging in again

Check Spam Folder: The email might be in your spam/junk folder. Move it to inbox for future emails.

Issue: "This Email is Not Registered"

The email address you entered doesn't have a HealthForms.io account.

Solutions:

  1. Check if you're using the right email (if you have multiple emails)
  2. Open a support ticket at https://support.healthforms.io to request access
  3. Sign up if your organization allows it

Issue: MFA Code Doesn't Work

Solutions:

  1. Make sure you're entering a fresh code (they expire every 30 seconds)
  2. Check your device's time is correct (authenticator apps depend on accurate time)
  3. Try waiting a few seconds for the app to show a new code
  4. Use a recovery code if you have one saved

Time Sync Problem: Authenticator apps rely on your phone/computer's clock being accurate. If codes don't work, check your device time.

Issue: Cloudflare Turnstile Not Working / Stuck

Symptoms:

  • Turnstile verification box doesn't appear
  • Turnstile verification never completes
  • "Turnstile failed" error message

Solutions:

  1. Refresh the page - Press F5 or Ctrl+R and try again
  2. Check internet connection - Turnstile requires a stable connection
  3. Disable browser extensions - Some extensions block Turnstile:
    • Temporarily disable all extensions
    • Try logging in again
    • Re-enable extensions one by one to find the culprit
  4. Clear browser cache - Press Ctrl+Shift+Delete and clear all data
  5. Try a different browser - Confirms if it's browser-specific
  6. Disable VPN - Some VPNs interfere with Turnstile verification
  7. Wait and try again - Turnstile servers might be temporarily slow

If still not working:

  • Contact support
  • Tell us your browser and any error messages
  • We can help troubleshoot or provide alternative login methods

Issue: "Something Went Wrong" (Generic Error)

Solutions:

  1. Refresh the page (F5 or Ctrl+R)
  2. Clear your browser cache (try a different browser to test)
  3. Check your internet connection
  4. Try again in a few minutes (the service might be temporarily down)
  5. Contact support if it keeps happening

Browser Tips

For Best Experience:

  • Update your browser to the latest version
  • Enable cookies (required for login)
  • Disable browser extensions that block scripts (temporarily)
  • Use a supported browser: Chrome, Firefox, Safari, or Edge

Browser Cache Issues: If login isn't working:

  1. Press Ctrl+Shift+Delete (Windows) or Cmd+Shift+Delete (Mac)
  2. Delete "Cookies and other site data"
  3. Refresh and try again

Security Best Practices

Do:

  • Use a strong, unique password
  • Enable two-factor authentication
  • Log out when you're done (especially on shared computers)
  • Update your password every 90 days
  • Be suspicious of login requests via email or text

Don't:

  • Share your password with anyone
  • Use "Remember Me" on public computers
  • Log in from unsecured WiFi without a VPN
  • Write your password on sticky notes
  • Use the same password everywhere
