What it is: The Signup page lets organizations create a HealthForms.io account to start managing health forms for their members.
Who uses it: Organization administrators or representatives who want to start using HealthForms.io.
Important: The signup page is for organizations, not individual end users. Individual users are invited by their organization's administrator — they do not sign up on their own.

The signup page itself shows this notice: "If you received an email requesting that you upload forms for you or a family member please use our invitation acceptance process."
Navigate to https://app.healthforms.io/account/signup to begin.
The signup creates a Starter Plan account which includes:

If you are not already signed into HealthForms.io, you'll also fill in a User Account section:
Email Address — Your email (this becomes your login)
Password — Must meet requirements:
Confirm Password — Re-enter your password
If you already have a HealthForms.io account, a Sign in with an existing user link is shown at the top. Click it to log in and attach your new organization to your existing account.
Check the checkbox to agree to the:
Accepting terms is required to continue.
Click the Continue button to submit.
After you complete signup, your organization account is created and you can:
You do not need to sign up here. Ask your healthcare provider or event organizer for your invitation link or code, then follow the invitation guide.
Click Sign in with an existing user at the top of the signup form to link a new organization to your existing account.
Q: How does a new organization sign up for HealthForms.io? A: Go to the HealthForms.io signup page and fill in your organization information (company name, address, contact details) and create a user account. The Starter Plan includes 5 members and requires no credit card.
Q: How do individual users get access to HealthForms.io? A: Individual users do not sign up on their own. They receive an invitation from their organization administrator. See the Invitation Guide for the acceptance process.
Q: Is a credit card required to sign up? A: No. The Starter Plan does not require a credit card to sign up.
