HealthForms.io Family Portal

The Family Portal is where participants manage their health forms. After accepting an invitation from an organization, you use the portal to fill out or upload required health forms for sessions such as camps, sports programs, or other events.

Getting Started

  1. Accept an invitation — You receive an invitation email from the organization running your event. Click the link in the email or use the invitation code at the portal.
  2. Link a family member — The invitation is tied to a specific family member (participant). You choose to create a new profile or link to an existing one.
  3. Submit forms — From the dashboard, open a session and complete or upload each required health form.

Top Navigation Bar

The header bar appears on every page and contains:

  • Hamburger menu icon — opens the right-side panel
  • Notifications bell — shows pending invitations

Right Side Panel

Click the hamburger icon to open the side panel. It contains:

Quick Start Guide

The Quick Start Guide in the side panel explains:

  • Adding Family Members — three ways to accept an invitation:
    1. Click the Get Started button in the invitation email (automatically links the profile)
    2. Log in, go to Invitation Acceptance, and paste the invitation code from your email
    3. Go to Pending Invitations in the portal and click Accept (if the invitation was sent to your login email)
  • Family Member Profiles — click a family member's name to view their profile, uploaded forms, and sessions
  • Support — link to https://support.healthforms.io

Common Issues

I received an invitation but I don't see any family members

Go to the side panel → Invitations, or click the Actions section on the Dashboard to find any pending invitations. Use the Invitation Wizard to accept the invitation and link the family member to your account.

I can't log in

Visit https://support.healthforms.io to open a support ticket.

Q: How do I get started with HealthForms.io? A: You'll receive an invitation email from the organization. Click the link in the email, log in (or create an account), and follow the steps to link the invited person to your profile. Once linked, you'll see their forms on your dashboard.

Q: Where do I enter my invitation code? A: Go to https://app.healthforms.io/portal/invitations/wizard to open the Invitation Wizard. On Step 1 (Enter Code), type the code from your invitation email and click "Submit Code." You can also reach the wizard from the Dashboard — click the "Pending Invitations" action card if one is shown, or open the side panel and click a pending invitation in the Invitations accordion.

Q: How do I log out? A: Click the hamburger menu icon in the top-right corner, then click "Logout" in the side panel.
