How to Accept a HealthForms.io Invitation

New users join HealthForms.io by accepting an invitation. This guide walks through the entire process.

Overview

What it is: An invitation is a link or code sent to you by a HealthForms.io administrator that allows you to create your account and gain access.

Who uses it: Anyone who has been invited to HealthForms.io by an administrator.

Why it matters: HealthForms.io does not allow open self-registration. Every user account starts with an invitation.

Two Ways to Accept

When an administrator invites you, you'll receive an email with a unique invitation link. Click the link — it will take you directly to the invitation acceptance page with your code pre-filled.

Option 2: Enter Your Invitation Code Manually

If you have an invitation code (from an administrator or event), go to https://app.healthforms.io/account/invitations and enter the code in the Invitation Code field, then click Accept Invitation.


Step-by-Step: Accepting an Invitation

Step 1: Open the Invitation

Click the invitation link in your email, or navigate to https://app.healthforms.io/account/invitations and enter your code.

Error messages you might see on the code page:

  • "Invitation Code Already Used" — Your code was already redeemed. If you already created your account, log in at the normal login page.
  • "The code used is either invalid or has already been used" — Double-check the code for typos and try again.
  • "The code used is invalid because the event has ended" — The invitation period has ended. Open a support ticket at https://support.healthforms.io to request a new invitation.

Step 2: Create Your Account

After a valid code is entered, you are taken to the Accept Invitation page.

The page message depends on which application you're joining:

For Portal users (family health forms):

Welcome! You're about to create a user account to manage health forms for your family. Please use your own email address to set up your user account.

For Manager users (staff):

Welcome! You're about to join the HealthForms.io Manager application. You will be required to set up multi-factor authentication (MFA) for your account with either a cell phone number or an authenticator app.

Fill in the form:

  • First Name
  • Last Name
  • Email — Use your own email address; this becomes your login
  • Password — Must meet these requirements:
    • At least 8 characters
    • At least one uppercase letter (A–Z)
    • At least one lowercase letter (a–z)
    • At least one number (0–9)
    • At least one special character

Confirm Password — Re-enter your password to confirm

Then click Create My Account.

Or use Google Sign-In — Click Sign in with Google to create your account using your Google credentials instead of a password.

Already have a HealthForms.io account? Click Already Have An Account to log in with your existing account and link it to this invitation.

Step 3: After Creating Your Account

Portal users: You are automatically redirected to the Portal app (https://app.healthforms.io/portal) where you can complete the process of accepting forms for your family.

Manager users: You are taken to the Account Security (MFA setup) page before accessing the Manager app.


MFA Setup for Manager Users

Manager users are required to set up Multi-Factor Authentication (MFA) before they can access the Manager application.

The Account Security page shows two options:

Method Description
Authenticator App (Recommended) Time-based codes from an app on your phone
SMS Text Messages Codes sent by text to your mobile number

A green shield checkmark appears next to a method once it's set up.

To set up MFA:

  1. Click Setup next to your preferred method
  2. Follow the setup steps (see MFA Setup Guide for details)
  3. Return to this page — you'll see the checkmark indicating it's active
  4. Click Finish Setup

Manager users cannot skip MFA. The Finish Setup button is disabled until at least one MFA method is configured.

Portal users see the same page but MFA is optional and the Finish Setup button is always available.


Completing Setup (Manager Users)

After MFA setup, you are shown the Success! Your Account is Set Up confirmation page.

From here:

  • Click Get Started to open the Manager application
  • Click My Profile to review your account settings

Common Issues

I Didn't Receive an Invitation Email

  1. Check your spam/junk folder for an email from HealthForms.io
  2. Add no-reply@app.healthforms.io to your contacts to prevent future filtering
  3. Open a support ticket at https://support.healthforms.io to request a new invitation

My Invitation Code Doesn't Work

  • Check that you've entered the code exactly as shown (no extra spaces)
  • The code input uses a masked format — make sure all characters are filled in
  • If you see "invalid or already used," the code may have been used on a previous account creation attempt — try logging in instead
  • If the event/period has ended, open a support ticket at https://support.healthforms.io

I Already Have an Account

Click Already Have An Account on the invitation page. This logs you in with your existing account and links it to the invitation.

I'm a Manager User and the "Finish Setup" Button is Disabled

You must set up at least one MFA method (Authenticator App or SMS) before you can finish. Click Setup next to either option.
