What is HealthForms.io?
HealthForms.io is a secure medical form management platform that keeps each participant's health information accurate, organized, and in one place. Organizations invite participants by email; participants create an account, link the people they manage, and submit the required health forms for each event or session — online or by uploading a photo or scanned PDF. Health staff then review each form for completeness and approve, request changes, or reject it.
What you can do
- Collect health forms for events, camps, programs, and activities — required forms are requested automatically per session.
- Submit forms your way — fill them out online, upload a scan or phone photo, or have a doctor fax a signed form directly to HealthForms.io.
- Keep emergency and insurance information current, so the latest details are on hand when they're needed.
- Review and track submissions — staff approve, request corrections, or reject, and everyone can see status at a glance.
- Control access — granular permissions limit who can see and act on each participant's information.
- Stay on schedule — automated notifications and reminders go out for upcoming deadlines and outstanding forms.
Your data is protected. HealthForms.io stores information in encrypted cloud storage and limits access to authorized users only.
