HealthForms.io offers two main methods to submit your forms:
Interactive Form – Fill out your forms digitally in your web browser. If a doctor's signature is required, you'll print specific pages, have them signed, and upload just those signed pages.
Upload Only (PDF or Image) – Directly upload your already completed forms as PDF or image files (PNG, JPG, JPEG).
Log in to your HealthForms.io account and go to your event dashboard.
Find the form listed under your event, indicated by the button labeled "Fill Out Form".
Click "Fill Out Form" to begin the interactive process.
The form will open in your web browser with interactive fields.
Enter all required information directly into the fields.
HealthForms.io will attempt to pre-fill your form with existing information, such as your name, date of birth, and health history.
If you've previously completed this form, it will pre-fill fields with the data you last entered.
You can save your progress as a draft to complete later or submit immediately.
If the form requires a doctor's signature:
To download a printable form to bring to your healthcare provider, click the Download Form button.
Bring these pages to your healthcare provider for their signature.
Return to your event dashboard and select the form again to complete the process. Verify that the information entered is correct, and click Upload Signed Pages. For instructions on how to upload the page(s), see the Upload Only (PDF or Image) section.
Drag and drop your PDF or image files onto the upload area or click "Upload Form" to select files from your device.
Pages awaiting upload will display an "Upload" button, with a faded preview of the expected page in the background.
After uploading, preview each page to confirm accuracy.
Use the buttons to the right of each page to rearrange, replace, add, or delete pages as needed.
Once satisfied with the uploaded pages, click "Submit" at the top of the page.
That's it!
Your forms have been successfully uploaded, securely stored, and made available to the event organizers.
